The page is numbered 238 and 37. I think that everything that is written inside is in Latin.

I do not think this page was taken from the original manuscript. 

More likely, it was copied in order to be of help to practicing doctors.

Think it is an original drawing, there are writings in Persian on it as well, especially inside the head of the figure.

Can make out on a small plate that it was manufactured in Waterloo, Iowa.

Do not have any other information and wasn’t able to locate anything with this information.
