Description

I have no idea what this is. I think the Chinese looking dogs and surround on the top may be Ivory. The handle is wood I think and the beads seem to be some sort of stone. On the end of the hanging beads is a stone skull.

Tibetan prayer wheel or rattle. That’s a guess but you might want to research in that direction.

MarkBlemish is correct - it is a Tibetan Buddhist prayer wheel. These wheels are also known as Mani wheels in the Tibetan language. They are used as an aid to meditation - the inner cylinder contains a tightly scrolled paper containing the full mantra (Om, Ma, Ni, Pad, Me, Hum). The idea is that spinning the wheel has the same effect as reciting the prayer (mantra) out loud. They are considered to be extremely powerful in prayer. Here's an example of one that isn't as ornate as yours: https://www.dia.org/art/collection/object/buddhist-prayer-wheel-72637
